HOMA-IR (Insulin Resistance Score)

HOMA-IR (Homeostatic Model Assessment of Insulin Resistance) is a calculated score using fasting glucose and fasting insulin together. It estimates how resistant your cells are to insulin's effects. The formula is (fasting glucose × fasting insulin) ÷ 405 (in mg/dL units).

Normal Range

Optimal: < 1.0; Acceptable: 1.0–1.9; Insulin resistance: 1.9–2.9; Significant: > 2.9

Reference ranges may vary slightly between laboratories. Always interpret your result in context with your doctor.

Why is HOMA-IR measured?

HOMA-IR is one of the most useful early-warning markers of metabolic dysfunction. It detects insulin resistance years before glucose, HbA1c, or weight changes appear abnormal — making it valuable for cardiovascular and diabetes risk prediction.

High HOMA-IR means…

HOMA-IR above 1.9 indicates insulin resistance — your body is overproducing insulin to keep glucose normal. Scores above 2.9 are significant and predict elevated risk of type 2 diabetes, fatty liver, hypertension, and cardiovascular disease.

Low HOMA-IR means…

A HOMA-IR below 1.0 indicates excellent insulin sensitivity. This is the metabolic state associated with the lowest risk of type 2 diabetes and cardiovascular disease.

Symptoms associated with abnormal HOMA-IR

When HOMA-IR is high:

  • Difficulty losing weight despite efforts
  • Energy crashes after high-carb meals
  • Persistent belly fat
  • Sugar cravings
  • Skin darkening at neck, armpits, or groin (acanthosis nigricans)
  • Irregular menstrual cycles

When HOMA-IR is low:

  • No symptoms — represents optimal metabolic health

How to improve your HOMA-IR

  • Resistance training is the most effective single intervention for HOMA-IR — even 2 sessions per week improves the score within 8 weeks.
  • Reducing added sugars and ultra-processed foods often drops HOMA-IR within 4–6 weeks.
  • A 5–10% body weight loss can halve HOMA-IR in many people.
  • Some people benefit from a low-glycaemic-index diet or time-restricted eating; trial different patterns and retest.

Frequently asked questions about HOMA-IR

Why is HOMA-IR more useful than glucose alone?

Insulin resistance can be present for 5–10 years before glucose climbs into the pre-diabetic range. HOMA-IR catches this hidden metabolic dysfunction early, when lifestyle change is most effective.

Is HOMA-IR available from my regular blood test?

It is calculated, not directly measured — your lab needs to report fasting glucose AND fasting insulin together. Not all routine panels include fasting insulin, so you may need to ask for it specifically.
